SCSI Read disk error - please reboot

Hey all,
I have just purchase an OLD Digital server (model
Has a PCI raid card (model DAC960)
I have set all drives up and everything looks ok. Diagnostics on the SCSI
card and drives come back fine.
I can install 2K server and run through the first install process that
copies files from the cd-rom to HDD's but upon first reboot I get this
error.
If I load a command file etc onto C it will boot into the command prompt no
worries.
I have been told it could be the ID's for the drives or perhaps RAM?

I have never delt with SCSI before but am very keen to learn,

What SCSI card? What hard drives? IBM has drive fitness diagnostic for

their drives. Looks like a dodgy drive.

The SCSI Card is a DAC960

Its a 3 channel card - 1 runs the cd-rom and the other two run the 4 drives.

I have tried installing 2K server with having all 4 HDD's on Raid 5 and also
tried 1 at a time using as a stand alone HDD - all options have the same
effect. I cant find a way to boot off the CD-ROM as well - BIOS doesn't
detect the card so it will only allow boot from floppy or HDD - the card
drivers are loaded after the BIOS.

Any thoughts would be greatly appreciated
